Standard Order Form
- A revised order form is attached that includes ChIQ™ and the CLENZIderm Peel Pads.
- Please replace any older versions you may have saved and use this form moving forward
- This updated version also has updated SKUs for some of the products
Skin & Syringe Solutions Promo Order Form
- A separate order form has been created specifically for accounts participating in the Skin & Syringe Solutions Promo.
- This version includes the excluded skincare products that qualify under the promotion and should be used when submitting these promotional orders.
Using the correct order form will help ensure orders are processed accurately and prevent delays.
☀️ New Summer Shipping Schedule (For Injectables Only)
- Continue submitting injectable orders as you receive them
- Orders will be processed as they are received and shipped according to the updated summer shipping schedule.
Summer Shipping Schedule :
Monday – Wednesday (before 3:00 PM ET)
- Orders will ship 2-Day
Wednesday (after 3:00 PM ET) – Thursday (before 3:00 PM ET)
- Orders will ship Overnight
Friday
- No injectables ship out on Friday
- Orders submitted on Friday will ship on Monday
🆕 New Account First Order Process (Updated Process)
Because injectable products can only ship Monday through Wednesday, there may be times when a new account's first purchase cannot be submitted as a single order if it includes both skincare and injectables.
If you're opening a new account and the first purchase includes both skincare and injectable products, please remember:
- Submit the skincare order and injectable order on the same day, even if they must be entered as two separate orders due to shipping requirements.
- This guidance applies only to new account first orders.
- Submitting both orders on the same day helps us accurately track first-order activity and ensures the account is reported correctly.

To support the launch of OBAGI® ChIQ™, we recently added a new Product Type drop-down field in Sugar
Effective immediately:
✅ Select the correct product when logging injectable samples distributed in Sugar:
- MagIQ
- ChIQ
Sampling both injectable products during the same office visit?
Please create two separate sample records in Sugar:
• 1 record for MagIQ
• 1 record for ChIQ
